What is a deep necklift?

A deep plane necklift, also known as a "weekend lift," is an advanced surgical procedure that focuses on improving neck contour and defining the jawline. This procedure is suitable for patients in whom aging has led to sagging in this area, as well as younger people who naturally have a fuller neck or a less pronounced jawline, regardless of aging.

Rather than being limited to superficial fat removal or skin tightening, this technique targets the deeper structures in the neck. This allows for structural, long-lasting and natural-looking results that fit each patient's unique anatomy.

Or rather a deep neck reduction?

For optimal results in defining the neck contour and jawline, in addition to lifting the deeper layers (as in a classic necklift), it is sometimes necessary to reduce excess volume in the neck. We do this with a deep neck reduction. Through a small incision of only 1.5 cm below the chin , we remove excess deep fat, correct the muscles and, if necessary, reduce the submandibular gland.

Thus, we restore, or create if naturally less present, the sharp angle between the chin and neck. The result is a tighter, youthful and refined neckline.

Who is an ideal candidate for a necklift?

Patients with a congenitally fuller neck or a less pronounced jawline are often excellent candidates for a Deep Plane Necklift. During this procedure, both the skin and deeper layers, such as muscles, are carefully released and repositioned to improve the contours of the neck and jawline.

Effects of a necklift

As we age, tissue elasticity decreases, leading to soft tissue sagging and tissue excess. The result is loss of definition in the neck and jawline.

In some cases, however, a heavy or poorly defined neck and jawline may be genetic and unrelated to aging.
In rare cases, this is due to significant weight loss, where the tissues are not sufficiently elastic to compensate for the loss of volume and the influence of gravity.

Weight gain, in turn, can also result in a fuller neck and less definition at the level of the neck and jawline. In both cases, a deep necklift can be a successful treatment.

Recovery

The recovery process after a necklift can vary depending on the specific procedure, the techniques used and the patient's individual reaction.

Some common aspects of the recovery process after a necklift include:

  • Immediately after surgery
    Immediately following your necklift, your face and neck will be placed in a relaxed bandage to minimize swelling and support the healing process. You will be monitored in the recovery room before being allowed to go home.
  • Swelling and bruising
    During the first two weeks after surgery, swelling and bruising are normal. These symptoms will gradually decrease as the weeks progress.
  • Pain and discomfort
    Pain and discomfort are to be expected. Dr. Van Genechten always prescribes pain medication.
  • Bandages and sutures
    The dressing is usually removed after the first night and may or may not be replaced. Sutures are removed according to a schedule that Dr. Van Genechten will discuss with you.
  • Limited activity
    During the first few weeks, it is recommended that you limit your physical activity so as not to interfere with the healing process. Avoid strenuous exercise and stooping.
  • Scars
    Although most scars after a face and necklift heal well and become less visible over time, scar treatment is still important. Dr. Van Genechten will advise you on the best way to best care for your scars.

A platysmaplasty is essential in a necklift

Maybe it all sounds a bit complicated: a necklift, a deep necklift and now platysmaplasty. We'll explain.

A platysmaplasty is a technique that is often part of a necklift and focuses on improving neck contours. While the necklift tightens the skin and underlying structures and removes excess fat, the platysmaplasty specifically targets the platysma muscle, the thin muscle in the neck that can sag due to aging. This can lead to visible, vertical or horizontal neck bands.

By limited trimming, repositioning and approximating this muscle, platysmaplasty not only strengthens the neck structure, but also improves the sharpness of the angle between your chin and neck. Combined with a necklift, this technique creates a tighter, more youthful and beautifully defined neckline.

Risks

It is important to emphasize that most patients who undergo a necklift or facelift experience no complications. Still, as with any surgical procedure, there are some risks inherent in the procedure.

Some of the risks often cited in the medical literature include:

  • Damage to the facial motor nerve
    This risk is associated with most facial procedures, due to the physical relationship and proximity of the nerve to the surgical area, and can potentially affect facial expressions.
  • Damage to facial sensory nerve
    This risk is associated with most facial procedures, due to the physical relationship and proximity of the nerve to the surgical area, and can potentially affect facial sensibility.
  • Infection and subsequent bleeding
    As with any surgery, there is a risk of infection and subsequent bleeding. However, strict adherence to sterile procedures and careful follow-up care significantly reduces the risk of these complications.
  • Delayed healing of wounds, de-epithelialization or partial necrosis of the skin
    Some patients may experience delayed healing, de-epithelialization (loss of surface layer of skin), or in rare cases, partial necrosis of the skin. Strict postoperative care, smoking cessation and follow-up can minimize these risks.
  • Suboptimal scar healing
    The outcome of scarring can vary, and although modern techniques are used to keep scars as minimal as possible, scar healing may be suboptimal in some patients.
  • Residual or existing, persistent asymmetry
    Decongestion always proceeds asymmetrically. Therefore, residual asymmetries may remain or existing asymmetries may persist, depending on various factors. The goal of facial rejuvenation surgery is always to rejuvenate without changing the face. Correcting asymmetries falls under the concept of change.
  • Sialocoele
    If deep neck reduction is performed along with your necklift, there is the possibility of a salivary collection at the level of the neck.

When does the swelling disappear?

It is very common to experience swelling after surgery. Remember that your body is healing from surgery and even pronounced swelling is normal.

It sometimes takes 4 to 6 weeks for the swelling to go away. Most swelling will disappear after the first 2 weeks.

The more active you are, the more swelling you may experience. The younger you are, the faster swelling disappears. The older you are, the longer swelling will be present.

Do I see blue after a neck elevator?

Bruising after neck lift surgery is frequent. You can cover any bruising with makeup after 3 weeks. Usually, all bruising disappears within the first 2 weeks.

In which position do I sleep best?

For minimal swelling, we recommend that you sleep on your back. However, if you feel more comfortable lying on your side, you may do so.

It does not affect the final result of your neck elevator.

When can I take a shower?

You may shower and bathe the first day after surgery if you gently clean the scars with baby shampoo and do not keep the incisions under water for too long.

Avoid too hot water to keep your blood pressure down.

Can I find good advice on the Internet?

It is very difficult to find good information on the Internet. Especially after your own surgery, asking Dr. Google or looking up advice on online platforms can lead to confusion or unnecessary worry.

How long will my face feel numb?

It is not unusual for the skin of your face and the skin around the ear to be less sensitive after a neck elevator.

This is caused by the skin being detached from the deeper muscle layer in certain places and the deeper muscle layer in itself from the deeper structures in the face. By definition, this causes the ends of sensory nerves to pass through. These need time to recover.

Recovery occurs as patchy or "patchy" and full recovery is sometimes not final until often between 1 to 1.5 years after the procedure.

What should I do if I have trouble sleeping?

Make sure your pain is well controlled throughout the day. Be careful about taking naps during the day. Try to plan your activities as normally as possible.

Is it normal to feel a pressure and tension?

It is not unusual to feel pressure or tension on your face or neck. Swallowing difficulties are also often present. These sometimes remain present for several weeks.

To relieve any pressure sensation, make sure you take your antibiotics and painkillers, sleep in an elevated position and avoid strenuous exercise.

What should I do if I think my wounds are infected?

As mentioned above, you will experience some bruising and swelling after surgery.

Contact us if your scar begins to give off yellowish or cloudy fluid.
If you experience new pain that is not under control with your pain medication, if you are constantly cold, have chills, or have a constantly elevated body temperature above 38°C, please contact us.

These symptoms do not necessarily mean that you have an infection, but these are symptoms that we should be notified of.

What should I do if I think I have skin necrosis?

Skin necrosis is a very unlikely event. In the unlikely event of necrosis, you will see a discoloration of your skin. First the skin will appear pale and then a dusky tint may appear.

If you think you see signs of discoloration of the skin next to your scar, do not hesitate to contact us and we will examine the skin as soon as possible.
